2. Cognitive Training and Rehabilitation Approaches for Addiction: A Delphi Consensus

Abstract

There has been important progress in the development of cognitive training/remediation interventions in the context of substance use disorders recently. However, there is a lack of standardized guidelines and some degree of skepticism about their usefulness in the field. This study aims to reach experts’ consensus on the best-suited intervention’s targets, techniques, active ingredients and modes of delivery in this regard.

 

A Delphi consensus was conducted by a steering committee (SC) of 15 members from the ISAM-NIG and 54 expert panelists (EP) recruited from performing a systematic review of the field. The SC developed the online questionnaire covering four domains as mentioned above in 67 items. A desirable agreement rate of at least 70% was set to be achieved through an iterative process within the EP (pre-registration link: https://osf.io/jwdtv/).

 

We achieved 64% response rates during the first iteration (n=54) with an agreement on 70.15% of the items. Twenty items reached partial agreement (>50 <70%) and were carried forward to the second iteration, together with 3 new items suggested 'de novo' by at least two experts. We have now completed the second iteration, with 96% response rates (n=52), and results will be analyzed shortly.

This study illustrates the capability to leverage both international networks and unbiased approaches (i.e., systematic review) to build an international network of experts with authority and onus to develop a consensus on the best approaches to develop cognitive training/remediation interventions in addiction. These interventions have mechanistic relevance and emerging evidence base, but need broadly-agreed standardized recommendations to be successfully translated in practice and policy.
